Rules and Regulations (the “Rules”)
Qualifying purchases must be made between June 1, 2019 and August 1, 2019.
Offer good while supplies last. Ply Gem Industries, Inc. (“Sponsor”) will provide each Qualifying Purchaser (as defined below) with a rebate in the form of either one (1) square of Ply Gem Cedar Shake siding or Ply Gem Cedar Shingle siding (collectively, the “Rebate Products”) for each qualifying purchase of eighteen (18) squares of Qualifying Products (as defined below) at a participating location of Crossroads Building Supply, Inc. (“Crossroads”). The Rebate Products will be awarded by Crossroad Building Products to Qualifying Purchasers, who meet all criteria as determined by Sponsor, in its sole discretion, within three (3) to six (6) weeks after the Sponsor has received and verified the submission. Sponsor makes no representations about the tax impact of the rebate. Consult a qualified tax professional to understand your tax obligations.
The following are products eligible for this Promotion (collectively, “Qualifying Products”):
- American Tradition siding
- American Herald siding
- American Splendor siding
- American Essence siding
Qualifying Purchasers are contractor customers, as solely determined by Sponsor. To redeem the offer, a Qualifying Purchaser must submit a legibly and fully completed redemption form together with original or legible copies of invoice(s) that show: (1) the total number of squares of Qualifying Products purchased, (2) the name of the seller, and (3) the date of purchase, which must be between June 1, 2019 and August 1, 2019. A properly completed redemption form, along with invoices must be received by August 15, 2019. Redemption forms must be completed with all requested information. A separate submission must be made for each purchase. Order acknowledgements, packing slips, pick tickets, quotes, or credit memos are not acceptable as proof of purchase. No submissions will be accepted after August 15, 2019.
Submissions must be made at the following participating locations of Crossroads: Albertville, AL; Beech Island, SC; Clanton, AL; Corinth, MS; Dothan, AL; Ethridge, TN; Fort Payne, AL; Hot Springs, AR; Lewisburg, TN; Lexington, TN; Little Rock, AR; Macon, GA; Martinez, GA; Mobile, AL; Muscle Shoals, AL; Oklahoma City, OK; Panama City, FL; Pensacola, FL; Rogersville, AL; Springdale, AR; and Tulsa, OK.
Sponsor is not responsible for lost, late, illegible, incomplete, mutilated, delayed, duplicate, altered, or misdirected submissions. Requests from P.O. boxes will not be honored or acknowledged. Sponsor reserves the right to audit accounts for compliance as well as Qualifying Purchaser’s purchase of product and submittal of redemption form and to disqualify Qualifying Purchasers engaging in deceptive or fraudulent submission practices. Duplicate, excessive or false submissions constitute fraud and may result in federal prosecution under the U.S. mail fraud statutes (Title 18, USC 1341 and 1342). All submitted materials become the property of Sponsor and will not be returned.
By completing and submitting the redemption form, you acknowledge and agree to Sponsor’s collection of your personal data in conformance with its privacy policy located at
https://www.plygem.com/wps/portal/home/aboutus/privacy-policy. Further, you agree to receive email notifications or other communications about special offers, events, new products and services from Sponsor and/or their affiliates.
If eligible Qualifying Products are returned after receipt of the Rebate Products, Qualifying
Purchasers will be required to return the Rebate Products or, if already used, the total cash value of the Rebate Products.
By submitting a redemption form, you agree to release and hold harmless Sponsor and its
respective affiliates, and their respective officers, directors, employees and agencies (collectively, the “Released Parties”) from and against any claim or cause of action arising out or in connection with this Promotion. You further agree that in any cause of action, the Released Parties’ liability will be limited to the costs of entering and participating in the program and in no event shall the Released Parties be liable for attorney’s fees. You waive the right to claim any other additional damages whatsoever, including but not limited to, punitive, consequential, direct or indirect damages. By participating in this Promotion, you agree to release and hold harmless the Released Parties from any and all liability for injuries, death or losses or damages to persons or property as well as claims and/or actions based on copyright or intellectual property infringement, misappropriation, data privacy rights, publicity rights, defamation, and/or invasion of privacy.
In the event of any discrepancy or inconsistency between these Rules and disclosures or other statements contained in any Promotion related materials, including, but not limited to, the redemption form, these Rules shall prevail, govern, and control. Sponsor reserves the right, in its sole discretion, to cancel, suspend, and/or modify this Promotion, or any aspect or part of it, for any reason whatsoever, including if any fraud, technical failures, or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of this Promotion. Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with any element of this Promotion or acts in violation of these Rules or in a disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of this Promotion may be a
violation of criminal and civil law, and should such an attempt be made, Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law.
Offer void where prohibited or restricted by law. Qualifying Purchasers agree to all terms of this Promotion. All reference to “squares” with respect to the Qualifying Products or Rebate Products shall mean a one hundred square foot (100 SF) of the applicable siding product. All decisions regarding the interpretation of the Promotion rules lie solely with the Sponsor, whose decisions are final and binding. Sponsor is not liable for injuries or losses arising from or resulting from participation in the Promotion, the Rebate Products, or items purchased therewith, and is not liable for events or errors by its employees or any acts or omissions of its employees, whether negligent or willful, in the conduct of this Promotion. Sponsor reserves the right to substitute rebate payments for the Rebate Products or modify or cancel this Promotion for any reason at any time. Qualifying Purchasers are responsible for all federal, state, and local taxes.
