Submitting your rebate is easy. Just follow these three easy steps.

1. Scan or take a photo of your invoice. Be sure we can see the purchase price, date of purchase, invoice number, and name of the company you purchased from.
2. Fill out the form and click choose file to upload the picture of your invoice.
3. Click Submit! It’s that easy!

Order must be placed between 7/16/18 – 11/1/18.
Rebate claims must be submitted by 11/15/18.
